Home Health Registered Nurse (RN) Full-Time

Position Overview
We are seeking a dedicated Full-Time Home Health Registered Nurse (RN) to provide comprehensive high-quality patient care in the home setting throughout Collin County. This role is responsible for conducting skilled assessments completing OASIS documentation using electronic systems (Kinnser preferred) developing plans of care and collaborating closely with physicians care teams patients and families.
Key Responsibilities
- Perform comprehensive in-home patient assessments including OASIS documentation and electronic visit notes.
- Evaluate the patients condition and home environment; develop and update individualized plans of care.
- Communicate effectively with physicians interdisciplinary team members patients families and referral sources to coordinate care.
- Serve as a patient advocate ensuring continuity and quality of care.
- Administer medications and prescribed treatments including infusion therapy wound care patient education and disease management.
- Provide direct supervision and oversight of LVNs and Home Health Aides (HHAs).
- Participate in case conferences in-services and ongoing quality improvement activities.
Qualifications & Requirements
- Current unrestricted Texas RN license.
- Minimum one year of geriatric home health experience.
- At least one year of clinical nursing experience.
- One year of OASIS experience required.
- Proficiency with healthcare software; Kinnser experience preferred.
- Strong organizational time-management and multitasking skills.
- Professional appearance with excellent verbal and written communication skills.
- Valid Texas drivers license and reliable transportation.
- CPR certification required.
- Ability to competently perform skilled nursing procedures as ordered by the physician and within the scope of the Nurse Practice Act.
- PICC line and Wound VAC experience preferred.
