Position Overview:
Full-time role in the Fostering Connections Program Child Assessment Center and Child Assessment Team Clinic. Seeking someone passionate about supporting children in out-of-home placements or who have experienced maltreatment. Role requires travel to Northern Lights Westside and Eastland locations. Prior experience with trauma-informed care is preferred. This position is Exempt and involves collaboration with a multidisciplinary team to provide child-centered care.


Job Description Summary:


Provides nursing care and care coordination services for pediatric patients and their families. Fosters effective communication among health care professionals who provide nursing care to children with altered health states. Develops and implements educational programs for patients parents and families hospital staff and community health personnel. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses.


Job Description:


Essential Functions:

  • Provides nursing care using a Family Centered approach.
  • Collaborates with primary care physicians specialty physicians and other service providers to identify patients in need of care coordination. Works closely with families to guide them through the health care system to optimize the care and services they receive to improve child health decrease costs and improve satisfaction with care.
  • Administers treatments in a manner that improves clinical outcomes and reduces risk or cost. Ensures delivery of preventive services health promotion activities and other interventions that proactively address potential risks. Aware of evidence based standards and applicable national standards.
  • Participates in policy and procedure development continuing education continuous quality improvement research and/or evidenced based practice.
  • Engages in the workplace community by staying aware of changes participating in data collection or auditing understanding Quality Improvement and representing staff at relevant meetings and then reporting important information and updates back to staff.


Education Requirement:

BSN required.


Licensure Requirement:

  • Licensed as a registered nurse in the State of Ohio


Skills:

Clinical skill and knowledge in the nursing care of children including normal growth and development and/or clinical expertise in specialized area required.
