What Youll Do
- Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
- Implement Therapeutic Interventions: Focus on functional strengthening gait training balance and fall risk reduction to improve patient mobility and independence.
- Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.
- Collaborate for Optimal Outcomes: Work with a supportive care team including other disciplines and supervisors to ensure coordinated patient care.
- Document Efficiently:Use one simple EMR system for all notes (with tools available to minimize documentation time!)
What You Need
- Valid California Physical Therapist license
- CPR or BLS certification
- Drivers license and auto insurance
- Physical TB test (within the last year)
- Professional liability and business license (if applicable)
