- The Supported Pathways Officer is a vital role within a local authority focused on providing assertive client-centered support to residents. This position is essential in guiding individuals along the housing pathway towards independent living. The officer will assist residents in maintaining their accommodation accessing necessary services and developing life skills that enhance their physical and mental well-being.
Responsibilities
- Deliver a customer-focused service with regular key working support planning and risk assessments for a caseload of approximately 10 residents.
- Support residents in maintaining their placements improving well-being and participating in meaningful activities that foster life skills and independence.
- Conduct interviews and assessments for new residents including safeguarding checks risk management assessments and health and safety inductions.
- Provide floating support for up to six months after residents move on to ensure tenancy sustainment.

- Working hours: 36 hours per week
- Enhanced DBS required
- Location: 259-263 Waterloo Road Greater London Southwark Surrey SE1 8JU United Kingdom
- Interviews will take place at Manor Place Hostel
- Bi-weekly payment schedule
