A fantastic opportunity has arisen to join Suffolk County Councils Bury Rural team as an Independence and Wellbeing Practitioner based at West Suffolk House Bury St Edmunds.

Your role and responsibilities

We are seeking an experienced enthusiastic practitioner with a positive solutionsfocused approach to join our friendly and supportive Bury Rural Neighbourhood Team in the west of the county.

In this role you will work closely with colleagues across the organisation and wider partners including health teams Allocation Leads Business and Finance Service Development and Contracts. This is a great opportunity to develop your skills and broaden your knowledge of Adult Social Care.

The core purpose of this role is to work with adults to identify and deliver highquality personalised outcomes that maximise independence and wellbeing.

You will:

  • work as part of a team to promote personalisation independence wellbeing and choice
  • complete strengthsbased assessments and reviews using an assetbased approach
  • create effective reablement plans using the Signs of Safety solutionfocused model
  • take part in the duty rota responding to urgent and emerging needs
