Full-Time Leadership Role

Northwest Talent Solutions (NWTS) is recruiting on behalf of a leading healthcare organization seeking an experienced Director of Case Management to lead hospital utilization management care coordination and transition management initiatives at a major acute care facility.
This leadership role is responsible for ensuring the effective use of hospital resources strong revenue cycle performance regulatory compliance and safe patient transitions across the continuum of care. The Director will oversee case management operations while working closely with physicians clinical teams and post-acute partners to improve patient outcomes and operational efficiency.
Key Responsibilities:
Lead the hospital utilization management program ensuring medical necessity compliance and denial prevention
Oversee case management department operations driving efficiency in patient throughput and care coordination
Implement and monitor the hospital Utilization Management Plan using data to drive performance improvement
Ensure accurate and compliant revenue cycle processes related to utilization management and case management
Guide patient transition planning and discharge coordination to support appropriate length of stay and readmission prevention
Collaborate with physicians and care teams to improve hospital utilization and care delivery processes
Lead initiatives to reduce payer disputes and reimbursement delays
Manage relationships with post-acute care providers and community care networks
Ensure compliance with CMS regulations accreditation standards and healthcare regulatory requirements
Provide education and guidance to physicians patients families and caregivers
Required Qualifications:
Bachelors degree in Business Nursing or Healthcare Administration (for RN candidates) or Masters in Social Work (MSW)
Active RN or LCSW/LMSW license in the state of practice
Minimum 3 years of acute hospital case management or healthcare leadership experience
Strong knowledge of utilization management patient throughput and hospital operations
Preferred Qualifications:
Advanced degree (MSN MBA MHA or MSW)
5 years of hospital case management leadership experience preferably across multiple sites
Accredited Case Manager (ACM) certification
Experience working with regulatory compliance accreditation standards and complex hospital systems
