About the Role
This rewarding opportunity will see you join the St Vincents Private Hospital Toowoomba (SVPHT) as ourClinical Nurse Consultant to undertake the role of Procedural Admissions Co-ordinator. This position is pivotal to the operational delivery of the hospitals Bringing Care Home initiative supporting the transition of appropriate surgical patients to short stay same day and home-based models of care.
Working in close partnership with the HITH Manager and Assistant Director of Perioperative Services this role will be a key liaison between surgeons anaesthetists practice managers and the multidisciplinary team to safely and efficiently integrate:
Short stay joint replacement pathways
Same day surgical procedures
Rapid discharge models supported by RITH (Rehabilitation in the Home)
HITH (Hospital in the Home) programs
The Procedural Admissions Co-ordinator will ensure patients are optimised pre-operatively appropriately screened for eligibility and seamlessly transitioned through theatre recovery inpatient (where required) and home-based care pathways.
This is a short-term contract position for three (3) months with potential extension dependent on initiative outcomes and service requirements.
Key Responsibilities
Act as the primary liaison between VMOs (surgeons and anaesthetists) practice managers perioperative services inpatient units RITH and HITH teams.
Coordinate and operationalise short stay joint and same day procedural pathways through theatres.
Support implementation of contemporary evidence-based models that safely shift care from inpatient settings to same day and home-based environments.
Collaborate with perioperative teams to optimise theatre utilisation and patient flow.
Review and screen patient suitability for short stay and home-based models in consultation with treating clinicians.
Facilitate pre-admission planning patient education and discharge coordination.
Monitor key activity and outcome metrics aligned with project targets.
Identify barriers to implementation and work collaboratively to develop solutions.
Ensure compliance with clinical governance safety and accreditation standards.

Things That Work for Us
Current registration with AHPRA as a Registered Nurse
Minimum 5 years post-registration experience including significant experience in two or more of the following:
Perioperative nursing (scrub/scout anaesthetics or recovery)
Orthopaedics (including joint replacement pathways)
Surgical short stay models
Project Management
Hospital in the Home (HITH) or Rehabilitation in the Home (RITH)
Demonstrated experience working collaboratively with surgeons and anaesthetists
Strong understanding of contemporary same day and short stay surgical models
Proven ability to coordinate complex patient pathways across multidisciplinary teams
Highly developed communication and stakeholder engagement skills
Demonstrated ability to influence change and support service redesign
Sound clinical assessment and risk screening capability
Strong organisational and time-management skills
Desirable:
Postgraduate qualifications in Perioperative Nursing Orthopaedics or relevant specialty
Experience in project implementation or service improvement initiatives
Knowledge of private hospital funding models and activity optimisation
