Role: Registered Nurse Case Manager (RN)
Location: Canonsburg PA
Position: Full Time Benefits & Sign on Bonus

Overview:

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team providing essential care and coordination for patients and their families.

Key Responsibilities:

Manage patient care in hospice settings.
Coordinate with healthcare teams.
Provide leadership and support to nursing staff.

Territory/Driving Expectation:

Canonsburg and 15-25 miles /- in a radius surrounding the community Schedule (Including any on call/weekends): 6 units daily; 30 units a week; 8 AM 5 PM Mon-Fri schedule. 1 weekend per month; 1 evening per week of on-call. Travel Local travel required covering multiple counties.
