Weekly Pay: $2678.45
13-Week Travel Assignment
Day Shift MondayFriday 8:00 AM 5:00 PM
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist for a 13-week home health assignment in Novato this role you will provide therapy services to adult and geriatric patients in their homes helping them regain mobility independence and functional ability following illness or injury.
Responsibilities

Evaluate patients and develop individualized treatment plans

Provide therapeutic interventions to improve mobility and functional independence

Educate patients and caregivers on exercises and home care strategies

Monitor progress and adjust treatment plans as needed

Document patient care and outcomes using the electronic medical record system

Collaborate with physicians and interdisciplinary care teams
Requirements
-
Graduate of an accredited Physical Therapy program
-
Active California Physical Therapist License
-
BLS Certification (AHA or American Red Cross)
-
Epic EMR experience required

Valid Drivers License and Automobile Insurance
-
Minimum 2 years of relevant experience (home health preferred)
Assignment Details

Home health setting serving adult and geriatric patients

Approximately 5 patient visits per day

Day shift schedule MondayFriday

Travel candidates must live 50 miles from the facility
