And we are looking for a Care Coordinator who thrives in responsibility.

The Role

As a Care Coordinator you are the heartbeat of operations.

If a shift goes unfilled families suffer.
If communication is sloppy trust erodes.
If schedules arent accurate the entire machine breaks.

Your job is to make sure that never happens.

What You Will Be Accountable For

  • Shift fulfillment Every shift covered every time

  • Maintaining schedule accuracy No surprises no confusion

  • Clear & timely communication With caregivers facilities and internal team

  • Building facility relationships Professional responsive trusted

  • Maintaining updated points of contact Always knowing who to call

  • Participating in operations calls & team alignment

  • 24-hour on-call rotation (shared among team)

  • In person client visits as needed

You will coordinate schedules based on client needs and caregiver availability ensuring shifts are filled promptly and accurately

You will serve as the primary point of contact for caregivers and facilities regarding assignments schedule changes and expectations

This role requires decisiveness urgency and ownership.

Schedule & Compensation

MondayFriday 8:30 AM 5:00 PM (30-minute break)
Weekly pay at $20/hour depending on experience

Full-time benefits including:
