Job Summary

The Registered Nurse (RN) Medical-Surgical / Telemetry (MS/Tele) is responsible for delivering high-quality patient-centered nursing care to patients with cardiac and stroke-related conditions in a fast-paced acute care environment. The RN assesses patient conditions develops and implements individualized care plans and collaborates with physicians and interdisciplinary healthcare teams to ensure optimal patient outcomes.

The RN must be skilled in telemetry monitoring dysrhythmia interpretation IV placement and phlebotomy as these competencies are required for nurses at this facility. The role requires strong clinical judgment the ability to respond quickly to patient condition changes and adherence to evidence-based nursing practices.

Key Responsibilities

Patient Assessment and Care

  • Perform comprehensive nursing assessments and continuously monitor patient conditions.

  • Develop implement and evaluate individualized care plans based on patient needs and physician orders.

  • Provide direct nursing care to patients with cardiac stroke and general medical-surgical diagnoses.

  • Monitor cardiac telemetry rhythms and identify abnormal patterns or dysrhythmias.

  • Respond promptly to clinical changes and initiate appropriate interventions.

  • Utilize rapid response teams when necessary for patient deterioration.

Clinical Procedures and Patient Management

  • Perform phlebotomy and peripheral IV insertion which are mandatory competencies for this facility.

  • Manage and maintain peripheral IV lines and ensure proper line care.

  • Administer medications and treatments as prescribed while monitoring for adverse reactions.

  • Provide oxygen therapy including nasal cannula oxygen masks and portable oxygen tanks.

  • Perform non-invasive airway management and monitor respiratory status.

  • Implement infection control protocols and maintain strict isolation precautions when required.

Telemetry and Cardiac Monitoring

  • Interpret cardiac telemetry and recognize dysrhythmias.

  • Manage dysrhythmia-related patient care interventions.

  • Monitor cardiac conditions and collaborate with physicians regarding treatment adjustments.

  • Document telemetry monitoring and patient responses accurately.

Documentation and Communication

  • Document all patient care activities accurately using the Epic electronic medical record (EMR) system.

  • Communicate patient status updates effectively with physicians nursing staff and interdisciplinary team members.

  • Participate in shift handoffs and care coordination to ensure continuity of care.

Collaboration and Support

  • Work closely with Patient Care Technicians (PCTs) who assist with vital signs blood glucose monitoring EKGs and activities of daily living.

  • Provide clinical guidance and support to assistive staff when necessary.

  • Assist with safe patient handling and transfers using equipment such as Hoyer lifts.

Patient Ratios

  • Day/Evening: 5:1 (may stretch to 67)

  • Night: 67:1 (may stretch to 810 if staffing shortages occur)

Patient Population

Education

  • Required: Associate Degree in Nursing (ASN)

  • Preferred: Bachelor of Science in Nursing (BSN)

Certifications & Licensure

  • Active New York State Registered Nurse (RN) License required

  • BLS Certification required

Experience Requirements

  • Minimum 1 year of Medical-Surgical or Telemetry nursing experience required
