Responsibilities
The Transitions of Care (TOC) Supervisor provides clinical and operational leadership for hospital-based TOC services across the health system. This role ensures standardized high-quality transitional care practices across multiple hospitals while supporting performance improvement interdisciplinary collaboration and alignment with system-wide goals related to quality safety and value-based care.
Qualifications
REQUIRED:
1. Currently licensed as a Registered Nurse/Advanced Practice Registered nurse in the state of Georgia
2. National Board Certification (e.g. FNP-BC AGACNP-BC).
3. Masters degree in nursing (MSN) or Doctorate in Nursing Practice (DNP) from an accredited school of nursing.
4. Credentialed by Northside Hospital Medical Staff office
5. Three (3) years of inpatient Transition of Care experience
PREFERRED:
1. Effective analytical and organization skills as well as excellent oral and written communication skills.
2. Demonstrated ability to educate others and serve as a positive change agent.
6. Leadership skills to direct others toward the successful treatment of patients.
7. Ability to assist staff in times of crisis or emergency.
8. Ability to determine work priorities assigns work and ensures proper completion of work assignments.
