This is for a Part Time PM Shift.
Through a holistic approach high quality comprehensive care is provided timely and efficiently to promote a positive end of life
experience in the PCU.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Conduct admission assessments
Establish plan of care in coordination with interdisciplinary team approach to collaborate revise update and assure services are provided according to established plan
Make assignments and supervise patient care provided by volunteers and staff members
Ensure physicians families and appropriate staff members are promptly notified of changes in patient condition
Make referrals to other services as needed in coordination with social workers
Educate families and patients on disease processes dying process treatment plan and assist them in making informed choices about care promoting independence
Provide active listening and support to patients and families through the trajectory of their disease progression
Provide services at the time of death including notification of staff per policy
Provide medications timely and accurately per physicians orders
Ensure safety of all residents implementing interventions needed as per plan of care
Ensure infection control policies and procedures are implemented and followed
Assure facility is secured per safety policies and procedures after hours and on weekends
Assure facility environment is appropriate to promote privacy and solace for all patients
Assure patient/facility laundry is appropriately cleaned and restocked
Other duties as assigned
KNOWLEDGE SKILLS & ABILITIES:
Able to work with the interdisciplinary team
Willingness to work with patients/families regardless of race color creed age or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptiveness
MINIMUM QUALIFICATIONS:
Education:
Current North Carolina license as a registered nurse
Experience:
Minimum 2 years nursing experience preferred
Licensures & Certifications: (if applicable):
CPR certification
CHPN within 3 years of hire
