Job Description:
- Assesses appropriateness of and completeness of prescriptions and medication orders.
- Selects compounds and dispenses drugs and other pharmaceutical products ordered by physicians and other licensed medical practitioners.
- Monitors drug therapy by evaluating patient medical history clinically significant drug interactions adverse reactions therapeutic duplication drug dosage and compliance with prescription order.
- Dispense and fill medication orders written by CHS medical and dental staff.
Requirements
Minimum Qualifications:
- Diploma/transcripts to support graduation with a Doctor of Pharmacy (PharmD) degree from a program accredited by the American Council of Pharmaceutical Education.
- Current valid unrestricted Pharmacist license issued by the Arizona State Board of Pharmacy.
- Current completed PSV from the Arizona State Board of Pharmacy.
- Minimum one year of past work experience in this position.
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
