A Brief Overview
Provides the full range of distributive clinical consultative research investigational and/or administrative pharmaceutical and medication safety services. Incumbents select compound manufacture purchase dispense and/or preserve drugs medicines and other therapeutic agents; serve as a member and consultant on treatment or diagnostic teams; disseminates drug information to members of the hospital staff and interested community groups; may participate in hospital research projects or perform pharmaceutical research.

What you will do
  • Clinical Pharmacy:Applies knowledge of solubility behavior of substances stability of drugs usages of drugs and limitations on modes of administering drugs and similarly applies knowledge of incompatibilities in evaluating dosages and permissible concentrations of drugs and in anticipating preventing and treating undesirable chemical knowledge of the physical and chemical properties and characteristics of computing dosages weighing and measuring drugs and mixing and compounding drugs and legal responsibility for proper interpretation of prescriptions and the dispensing of prescribed products and call to physicians attention any synergistic antagonistic and cumulative effects as well as questionable overdoses or fine differences between drugs as they relate to therapeutic efficacy of the continuous awareness for new developments in the fast-moving drug field and be aware of emerging patterns systems and techniques in pharmacy records documents patient data prepares written reports and provides oral reports concerning patient contribute to the accredited teaching programs indoctrination and refresher training proper storage protection and preservation of therapeutic agents in accordance with the standards of the United States Pharmacopoeia (U.S.P.) and the National Formulary (N.F.)
  • Investigational Drug Service:Provides assistance in the coordination and maintenance of pharmacy services relating to research at in sponsor site qualification and initiation practices relating to pharmacy dispensing based on IRB approved clinical guidance to investigational drug service pharmacy request attends various meetings to provide pharmacy input on drug as the liaison between the study team protocol sponsor and nursing to coordinate drug in audits from the sponsors FDA DEA NIH and other regulatory agencies as needed.
  • Medication Safety Officer Duties and Responsibilities:Develop and oversee the planning creation review and refinement of a medication safety develop and lead implementation of error prevention strategies based on practice standards best practice guidelines (or recommendations) literature review external error reports medication safety tools and analysis of the organizations medication safety medication management and patient safety are core components of overall organization quality and outcomes in the planning design and implementation of the organizations medication-use technology and automation a culture of safety through lesson learned education and communication across the entire processes to collect information on the organizations medication errors and system failures to ensure that they are captured and barriers to reporting are compliance with state and federal regulatory and legal requirements relating to medication safety and assist in the accreditation process by ensuring that the organizations medication-use processes meet applicable medication management standards and NPSGs.
  • Medication Safety Expert Coordinator: Serve as an authoritative resource on medication safety for the the medication safety perspective for technology the medication safety perspective to internal and external emergency-preparedness as an internal consultant to investigate medication safety events or issues and develop recommendations for as the chair of the medication safety committee whose duties may include setting the agenda reviewing general and specific error reports and examining the progress of projects and initiatives assigned to the medication safety knowledgeable in the application and use of a variety of quality-improvement methodologies and tools (e.g. FOCUS-PDCA or Lean methodologies RCA FMEA).Collect review and analyze the organizations medication- use process medication errors adverse drug reactions and continuous quality-improvement dataPeriodically review and update clinical decision support tools to alert staff to high-risk situations and educate staff as needed.
  • To further research and education regarding Medication Safety: Design and assist in the implementation of education and orientation programs in safe medication use including development of competency assessment for: Staff tasks related to medication safety (e.g. use of smart pumps and automated medication dispensing systems);Education of health care providers other pertinent staff and (as possible) patients to ensure they are competent in safe medication-use practicesProvision of effective ongoing programs and presentations related to safe medication use to diverse audiences (e.g. nursing pharmacy respiratory care and medical staff)Share information about actual or potential medication errors or harm with safety organizations such as ISMP FDA drug or product manufacturers and state error reporting medication-use safety research through well designed externally validated studies and implement evidence-based practices for medication to the literature on medication medication safety education to pharmacy colleagues students and residents as well as other health care medication safety into orientation and training for all health care providersEmployee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital. Education Qualifications
  • Bachelors Degree Pharmacy Required

Experience Qualifications
  • 2 years to 3 years of progressively responsible and directly related work experience. Once (1) year of completed a PGY1 residency. Required

Required Knowledge Skills and Abilities
  • Ability to apply judgment and make informed decisions.
  • Ability to communicate effectively both orally and in writing.
  • Ability to foster effective working relationships and build consensus.
  • Ability to plan organize prioritize work independently and meet deadlines.
  • Knowledge of influence of patients age sex concomitant disease states concurrent drug therapy foods and diagnostic procedures on drug activity and disposition.
  • Knowledge of local state and federal regulatory requirements related to areas of functional responsibility.
  • Knowledge of Medical terminology and abbreviations; pharmacology and pharmacological nomenclature; medical therapeutics pharmacokinetics and disease states.
  • Knowledge of methods and procedures involved in proper handling and storing of pharmaceutical supplies.
  • Knowledge of physiology anatomy pathology chemistry and mathematics as they relate to pharmacy practice.
  • Knowledge of prescription and non-prescription drugs and drug products and their indications efficacy dosage mechanisms of action; fate and disposition (if known) major contraindications and potential side effects.

Licenses and Certifications
  • PharmD - Doctor Of Pharmacy California Licensure required
  • Current American Heart Association Certification for Basic Life Support for HealthcareProviders required

Base Pay Scale: Generally starting at $75.21 - $100.96 per hour

The salary of the finalist selected for this role will be set based on a variety of factors including but not limited to internal equity experience education specialty and training. This pay scale is not a promise of a particular wage.
