Job Summary

The Health Home targets populations who are in Shepard Pratts day programs Assertive Community Treatment (ACT) program or residential rehabilitation programs. The team provides an extra layer of care that treats the whole person by identifying gaps and barriers to treatment.

What to expect.

This is an opportunity to coordinate somatic healthcare in tandem with mental illness emotional disturbance and/or substance use disorder treatment. You will provide advisory and advocacy for program staff and clients. This position will split time between Columbia MD and Glen Burnie MD spending at least 2 days per week at each location.

Specific responsibilities include:

  • Participating in initial treatment planning and development of associated goals.
  • Ensuring clients have access to medical providers and hospitals including coordination of admission and discharge.
  • Ensuring clients complete all required assessments screenings and immunizations.
  • Training staff on medical diseases treatment and medication.

We are searching for someone who has excellent interpersonal skills and a desire to develop connections with mentally ill clients and their family members.

What we need from you.

  • Must be a Maryland Registered Nurse at time of application.
  • A drivers license with 3-points or less and access to an insured vehicle.
