Job Summary
We are seeking an experienced Registered Nurse Case Manager for a 13-week travel assignment in an acute care hospital setting. This role focuses on care coordination utilization review and discharge planning to support safe and efficient patient transitions across the continuum of care.
The Case Manager RN will collaborate with interdisciplinary teams to ensure patients receive appropriate care resources and timely discharge planning while meeting clinical and regulatory requirements.
Assignment Details- Job Title: Registered Nurse Case Manager
- Location: Clinton MD
- Contract Duration: 13 weeks with possible extension
- Shift: 8:00am to 4:30pm
- Schedule: Day Shift 5x8 hours (8:00 AM 4:30 PM)
- Weekend Requirement: Every other weekend (2 weekends within a 6-week rotation
- Perform inpatient case management and coordinate care for hospitalized patients.
- Conduct discharge planning and ensure safe transitions to home or other care settings.
- Perform utilization review and evaluate patient care using InterQual and Milliman guidelines.
- Complete admission and continued stay reviews to determine appropriate level of care.
- Coordinate care with physicians nurses social workers therapists and other healthcare professionals.
- Manage daily patient caseloads of up to 20 patients while maintaining quality documentation.
- Provide patient and family education regarding care plans discharge instructions and available resources.
- Facilitate coordination with community resources and post-acute care providers.
- Ensure compliance with hospital policies CMS regulations and quality standards.
- Active Registered Nurse license (Compact or State license required)
- BLS certification
- Minimum 2 years of RN experience
- Travel experience required
- Acute care hospital experience required
- Strong experience with discharge planning and inpatient case management
- Ability to manage high patient caseloads efficiently
- Community hospital experience
- Experience with InterQual and/or Milliman utilization criteria
- Background in ICU medical-surgical step-down or womens services
- Prior utilization review or care coordination experience
