Position Summary
The Non-Clinical Post-Acute Care Case Manager serves as a liaison between healthcare systems post-acute facilities providers patients and families to support smooth transitions of care optimize patient experience and drive encounter volume within skilled nursing and assisted living facilities. This role focuses on coordination communication documentation oversight and census support without providing direct clinical care.
The Case Manager ensures operational efficiency from admission through discharge while strengthening facility relationships and supporting revenue growth initiatives.
Key Responsibilities
Care Coordination & Transition Support
- Serve as primary non-clinical point of contact for assigned SNFs and post-acute partners
- Facilitate communication between facility staff providers and operations teams
- Track new admissions and ensure timely provider assignment
- Support discharge planning coordination and follow-up scheduling
- Monitor length of stay and identify opportunities for encounter optimization
Census & Volume Growth Support
- Maintain visibility into daily census across assigned facilities
- Track new admissions payer mix and encounter frequency
- Collaborate with Business Development and providers to increase appropriate patient encounters
- Identify barriers to provider utilization and escalate appropriately
- Support initiatives to increase new admissions and facility penetration
Operational Oversight
- Ensure required documentation workflows are completed
- Monitor onboarding of new facilities or providers within territory
- Assist with resolving scheduling gaps or communication breakdowns
- Support Medicaid/Medicare documentation follow-up (non-clinical oversight only)
- Maintain accurate reporting in CRM or internal tracking systems
Relationship Management
- Conduct regular facility check-ins (in person or virtual)
- Build strong relationships with administrators DONs social workers and discharge planners
- Identify service concerns and escalate to leadership
- Support retention of facility contracts and preferred provider status
Qualifications
- Bachelors degree preferred (Healthcare Administration Business or related field)
- 2 years experience in post-acute care healthcare operations or care coordination
- Skilled nursing facility experience strongly preferred
- Strong communication and relationship-building skills
