**Core Position Responsibilities:**

**Referral Coordination:** Facilitate appropriate referrals to community resources and governmental agencies to optimize patient services.
- **Psychosocial Assessment and Treatment Planning:** Conduct comprehensive assessments of patients and their families analyzing psychosocial functioning needs strengths weaknesses coping mechanisms and required assistance. Collaborate closely with patients families and interdisciplinary teams to formulate and implement targeted treatment plans.

**Diverse Client Engagement:** Carry out psychosocial assessments and deliver treatment to a broad spectrum of individuals across varying socio-economic cultural and educational backgrounds.

**Clinical Knowledge Application:** Utilize knowledge of medical and mental health conditions including acute chronic and traumatic illnesses medications and their pharmacokinetics and relevant medical terminology.
- **Therapeutic Intervention:** Employ a range of treatment modalities tailored to individuals families and groups facing psychiatric medical and social challenges striving to meet established treatment objectives.
- **Counseling and Psychotherapy Delivery:** Provide professional counseling and psychotherapy services to individuals groups couples and families ensuring practice within the limitations of licensure.

**Consultative Support:** Offer consultation to interdisciplinary staff regarding the psychosocial dimensions of patient care and the implications of psychosocial issues on health outcomes and treatment adherence.
- **Professional Collaboration:** Participate in professional peer review case conferences research initiatives and other organizational roles mandated by the Medical Treatment Facility (MTF).

**Interdisciplinary Operations:** Engage in the clinics interdisciplinary framework which includes activities like scheduling coordination email communications facilitating warm hand-offs and participating in team huddles.
**Work Schedule:**Monday to Friday encompassing 9 hours worked between 7:30 AM and 4:30 PM with a rotating limited call schedule and callback requirements. No weekend or holiday shifts unless on-call.
**Minimum Qualifications:**

**Education:** A Master of Social Work (M.S.W.) degree from a Council on Social Work Education (CSWE) accredited institution is required.

**Experience:**
1. A minimum of one year within the past three years in evaluating and treating patients with pain and psychological factors influencing their physical conditions or coping with medical challenges.
2. At least 12 months of direct experience in the last 24 months involving computer operations basic word processing data entry and the use of electronic medical records.

**Licensure:** Possession of a current full active and unrestricted LCSW license from any state.

**Life Support Certification:** Must have and maintain a current AHA or ARC Basic Life Support (BLS) Healthcare Provider certification with the expectation to obtain it prior to the start date if not already certified.

**Security Clearance:** Capability to successfully pass a government background check and achieve requisite security clearance.
