Community Support Professional

All Racker CSPs working at least 10 scheduled hours per week will earn $18 per hour! Paid Mileage! All those working fewer then 10 scheduled hours per week will earn $17.25 per hour. All Hours Available!

Were looking for fun creative and flexible staff to provide community based support services for individuals with developmental disabilities of all ages. This position can be full time part time after school hours weekend hours . Community Support Professionals work one on one with individuals on their caseload to implement unique services to explore and build skills which support increased independence inclusion and community terms of work locations listed above you have the option of choosing the towns you prefer with a flexible daytime work schedule! This position requires that you have your own transportation. Current needs Waverly Barton Nichols Candor Newark Valley and Tioga Center.

Essential Duties:

• Responsible for the health and safety of the individuals supported
• Provide individualized services to people with disabilities and their families which may include: respite for families teaching modeling and supporting skill development in a variety of areas at home and in the community
• Participate as an active member of the persons team providing ideas input and advocacy for the person related to his or her Staff Action Plan
• Follow objectives as described in the persons Staff Action Plan and as discussed with supervisor

• Attend training to improve services provided
• Complete job requirements (training PPD etc) within appropriate time frames
• Report any potential incident to the designated Family Support Coordinator/Supervisor immediately or the day that it occurred.
• Accurately enter time and services into Rackers electronic health record eVero documenting service times supports provided and mileage according to program and regulatory requirements (e.g. contemporaneously documenting service provision)
• Use ofsmartphone for documentation purposes . Ownership/usage of personal smartphone is strongly preferred

• Communicate regularly with supervisor regarding service provision
• Attend scheduled supervisory meetings regularly
• Uphold standards of professionalism with regard to behavior and dress
• Assume responsibility for related duties as or assigned

• High school diploma or GED
• Ability to meet the agencys approved driver criteria (3 years experience with a drivers license and no more than 4 points)
