What does a Discharge Planner - State Hospital do

As a full-time Discharge Planner at the Hampton-Newport News Community Services Board you will play a pivotal role in facilitating patient discharges from State Hospitals ensuring smooth transitions for clients returning to the community. Youll consult with staff from Eastern State Hospital Central State Hospital or local psychiatric facilities to coordinate comprehensive discharge and after-care planning. The position entails conducting mental status evaluations and client assessments to make informed treatment recommendations and develop tailored service plans. Acting as a liaison among various facilities you will advocate for clients monitor their behavior and provide crisis intervention as necessary.

Additionally youll participate in the allocation of consumer funding and represent the agency on the Regional Discharge Assistance Committee (REDAC). Your involvement will extend to training program staff collaborating with law enforcement and health providers and implementing policy changes to enhance service delivery. This dynamic role is essential in promoting a seamless support system for clients in need.

What are we looking for in a discharge planner

To thrive as a full-time Discharge Planner at the Hampton-Newport News Community Services Board candidates must possess a robust skill set tailored to the unique needs of the client population served. A thorough knowledge of interviewing techniques and clinical assessment skills is essential for effectively evaluating client needs. Familiarity with community resources mental health and substance abuse case management programs and the Medicaid State Plan Option will empower you to develop comprehensive treatment plans.

To qualify for this position candidates must have the following:

Bachelors Degree in Human Services such as Social Work Psychology Sociology or Counseling.
Three (3) years of direct care experience in case management and counseling services to the client population assigned.
Valid Drivers License. Transports clients as necessary.

Additionally an understanding of counseling practices and ethical considerations is critical. Candidates should be skilled in crafting individualized behaviorally specific treatment plans and maintaining effective rapport with clients. The ability to observe interpret behavior and formulate accurate diagnostic impressions using the DSM IV will enhance treatment outcomes. Strong communication skills are required for preparing documentation and reports while effective collaboration with diverse stakeholders is key in addressing the total spectrum of service needs and ensuring a seamless transition for clients.
