0900-1700 M-F no weekends no holidaysResponsible for the delivery of professional nursing services which will address the physical psycho-social spiritual cultural developmental and age-appropriate needs of each individual patient/family. Provides care with a focus on discharge preparation and instruction and coordinates discharge planning with other disciplines.Responsibilities and Essential Job Functions
- Performs patient assessment
- Utilizes plan of care
- Validates that patients have clear and concise information and understanding of discharge instructions
- Completes accurate discharge medication reconciliation
- Assesses patient/family learning needs
- Utilizes interdepartmental collaboration according to patient needs to prepare for discharge
- Provides staff education on discharge process
- Ensures safety of patients visitors colleagues and self
- May perform Staff Nurse duties as needed
- Must be able to perform the professional clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Graduate from an accredited nursing program.
Preferred Education and Experience
- Bachelor Degree Nursing
Required Licensure and Certification
- Licensed Registered Nurse (LRN) - Single State - State Board of Nursing with eligibility to work in the state of MO.
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Licensure and Certification
- Medical/Surgical (RNC)
- Gerontology (RNBC)
- Progressive Care Nursing (Adult) (PCCN) - American Association of Critical-Care Nurses Certification Corporation (AACN)
- Certified Medical-Surgical Registered Nurse (CMSRN) - Academy of Medical-Surgical Nurses (AMSN)
- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA)
