• Performs patient assessment
• Utilizes plan of care
• Validates that patients have clear and concise information and understanding of discharge instructions
• Completes accurate discharge medication reconciliation
• Assesses patient/family learning needs
• Utilizes interdepartmental collaboration according to patient needs to prepare for discharge
• Provides staff education on discharge process
• Ensures safety of patients visitors colleagues and self
• May perform Staff Nurse duties as needed
• Must be able to perform the professional clinical and or technical competencies of the assigned unit or department.

• Graduate from an accredited nursing program.

• Bachelor Degree Nursing

• Licensed Registered Nurse (LRN) - Single State - State Board of Nursing with eligibility to work in the state of MO.
• Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)

• Medical/Surgical (RNC)
• Gerontology (RNBC)
• Progressive Care Nursing (Adult) (PCCN) - American Association of Critical-Care Nurses Certification Corporation (AACN)
• Certified Medical-Surgical Registered Nurse (CMSRN) - Academy of Medical-Surgical Nurses (AMSN)
• Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA)
