Position Overview
Provide comprehensive outpatient primary care services across the lifespan
Manage acute chronic and preventive health conditions
Perform annual wellness visits and health screenings
Coordinate care with specialists and hospital-based providers
Utilize EMR for timely and accurate documentation
Participate in shared call coverage (phone-only minimal burden)
Collaborate with advanced practice providers and support staff
Opportunities to participate in academic and quality improvement initiatives
Qualifications
Board Certified or Board Eligible in Family Medicine
Eligible for Connecticut State medical licensure
Active or eligible DEA registration
Strong clinical and communication skills
Commitment to patient-centered team-based care
