Job Summary

What were looking for:

CNS Healthcare is looking for Community Health Specialists to support our Behavioral Health Home (BHH) program by providing community-based integrated physical and behavioral healthcare services to individuals who may have complex and chronic mental illness substance use and medical co-morbidity in a fast-paced interdisciplinary team setting. This position will work with individuals served to enhance access to services and work to achieve goals for mental and physical wellness.

Were looking for candidates with priorexperience as a medical assistant community health worker being a recipient of mental health services. We will provide you with the training and support to become successful in this role and secure certification as a certified Community Health Worker Certified Medical Assistant or Certified Peer Support Specialist.

Compensation Range:

The starting pay for this position is $17.00/hour- $23.00/hour and is based on non-discriminatory factors such as skills and experience.

  • Coordinate and provide access to individual and family supports including referral to community social supports.
  • Meet regularly with individuals served in office and community settings.
  • Meet regularly with the care team to plan care and discuss cases and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinic.
  • Identify community resources (i.e. social services workshops etc.) for patients to utilize and maximize wellness.
  • Conduct referral tracking
  • Coordinate and provide access to chronic disease management including self-management support
  • Implement wellness and prevention initiatives
  • Facilitate health education groups
  • Provide education on health conditions and strategies to implement care plan goals including both clinical and non-clinical needs
  • Complete all required documentation completely accurately and in accordance with CNS
    policy
  • Develop and implement Person Centered Plans for Individuals consistent with guidelines
    established by OCHN DWIHN and other funding and accrediting entities
  • Establish collaborative relationship with Individuals
  • Utilize motivational interviewing strategies and a trauma-informed approach to engage with individuals and deliver services
  • Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served guests and staff.
  • Participate in and promote departmental and agency quality improvement initiatives.
  • Maintain knowledge and compliance with established policies and procedures corporate compliance program code of ethics applicable federal state and local laws and regulations HIPAA standards and other regulatory programs.

Qualifications:

  • High School Diploma or General Education Diploma (GED)
  • Valid and unrestricted Michigan Drivers License
  • Reliable transportation and current proof of auto insurance
  • Prior experience as a medical assistant community health worker OR being a recipient of mental health services
  • Intermediate level experience with computers and Microsoft office products
  • Ability to work closely with persons with mental or physical limitations
  • Ability to report and record information regarding individual dreams and desires verbally and in writing
  • Ability to communicate effectively professionally and courteously
  • Ability to use discretion and judgmental when handling matters of a sensitive or confidential nature
  • Intermediate time management problem solving customer service interpersonal and conflict resolution skills.
  • Intermediate presentation facilitation collaboration and team working skills.
  • Ability to be forward thinking and take initiative to accomplish goals and objectives of the department

Preferred

  • Experience working with persons who are mentally ill in either a hospital or community setting
  • Certification as a state-certified peer support specialist (CPSS) medical assistant or Community Health Worker
