Physical Therapist (PT) Part-Time or Full-Time

Birkwood Village/Montrose Health Center

We are seeking a dedicated resident-centered Physical Therapist (PT) to join our therapy team. This role is ideal for a PT who values meaningful relationships interdisciplinary collaboration and helping older adults maintain mobility independence and quality of life.

Role Overview

The Physical Therapist provides skilled evaluation treatment planning and therapeutic interventions for residents in our long-term care and assisted living community. Youll work closely with our therapy team nursing staff and families to ensure each resident receives individualized compassionate care.

Key Responsibilities

  • Complete PT evaluations and develop individualized plans of care
  • Provide direct therapy services to support mobility strength balance and functional goals
  • Supervise and collaborate with Physical Therapist Assistants (PTAs)
  • Document assessments progress notes and care plans accurately and timely
  • Participate in interdisciplinary care conferences and collaborate with nursing and other departments
  • Educate residents families and staff on mobility strategies and safety
  • Promote a positive encouraging environment aligned with our mission and values

Qualifications

  • Current Physical Therapist (PT) license in the state of Iowa
  • Experience in long-term care or geriatrics preferred but not required
  • Strong clinical reasoning communication and organizational skills
  • Commitment to resident dignity safety and well-being
  • Ability to work independently and as part of a collaborative team
