Intensive Family Coordinator

Status: Part Time Hourly FLSA classification: Non-Exempt

Summary of the Position:

• The Intensive Family Coordinator will be responsible for completing family assessments and creating individualized service plans.
• The Intensive Family Coordinator will report to the Director of the Program. Successful applicants will possess outstanding people skills creativity and energy as well as expertise of Tompkins County.

• Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families.

• Intensive Family Coordinator will be responsible for completing family assessments recruiting foster parents completing home studies for the referring authority locating family resources and creating individualized service plans.

• Case management services including intakes safety planning team meetings ISP development 24/7 support case contacts and case record documentation in keeping with state regulations.

• Travel will be necessary
• Flexible Schedule with willingness to work evenings and weekends and provide 24/7 crisis support to the caseload as needed.

Hourly Rate $22.00 per hour

Qualifications/Requirements:

• High School Diploma or Equivalent is required.
• Bachelors degree in social work psychology human services or related field is preferred.
• At least 1-2 years of experience working with children youth and families in social services behavioral health child welfare or community-based support programs.
• Experience providing case management family support crisis intervention or coordination of services is strongly preferred.
• Strong organizational and time management skills
• Proficient in Microsoft Office 365; knowledge of electronic health record (EHR) systems is a plus.
• Position requires reliable transportation valid drivers license and current auto insurance coverage.
• Bilingual (Spanish) speaking is a plus
