Responsibilities

• Provide direct support and guidance to individuals in the community who are experiencing mental health or substance use challenges.

• Meet clients where they areat home in the community or wherever support is needed.

• Develop and maintain strong trusting relationships with clients.

• Collaborate with interdisciplinary teams to create person-centered care plans.

• Document interactions progress and outcomes in accordance with agency policies and regulations.

• Advocate for clients to ensure access to resources and services.

Qualifications

• Bachelors degree in human services social work or a related field preferred.

• Experience working with individuals experiencing mental health or substance use challenges is highly desirable.

• Excellent communication problem-solving and interpersonal skills.

• Ability to work independently in community settings.

• Valid drivers license and reliable transportation required.
