  • POSITION DESCRIPTION

    Huron Valley Schools is seeking a Transition Coordinator who values collaboration and is well-versed in best practices within special education transition services. This individual will support districts educators and families at the secondary level (middle school high school and adult transition) to achieve meaningful post-school outcomes through regulatory compliance and the utilization of best practices. The selected candidate will demonstrate success in building relationships and in building futures one student at a time.

    MINIMUM QUALIFICATIONS

    • Masters degree from an accredited university in education or a field of study related to special education
    • Valid Michigan Teaching Certificate in Autism Spectrum Disorder (SV) Emotional Impairments (SE) Cognitive Impairments (SA) and/or Learning Disabilities (SM).OR fully licensed in a related field (school psychology speech language pathology school social work occupational therapy etc.)
    • Michigan Department of Education (MDE) Transition Coordinator Full Approval or ability to obtain
    • Minimum of five (5) years of professional experience as a special education teacher in a vocational rehabilitation role or in a related field.
    • Minimum of three (3) years experience Completing Transition sections of an IEP
    • Minimum of three (3) years experience training Paraeducators and/or Peers
    • Demonstrated ability to work cooperatively and confidentially with general and special education teachers administrators parents and support staff members.
    • Valid Michigan drivers license and the ability to travel to local schools job sites and community businesses throughout Oakland County
    • Basic knowledge of CTE EDPs Personal Curriculums and CBI
    • Basic understanding of community mental health and vocational rehabilitation systems
    • Demonstrate ability to work with high stress situations and have a strong ability to seek creative solutions.
    • Strong written and verbal communication skills to lend toward solution seeking with staff parents and advocates on behalf of students and the district.

    ESSENTIAL FUNCTIONS

    • Regulatory Compliance: Establish implement and maintain policies and procedures to ensure district compliance with Federal State and Oakland County regulations related to transition aged students with disabilities.
    • IEP Development:Assist train and monitor the completion of the Transition Services Component of the IEP including the development of measurable postsecondary goals.
    • Agency Collaboration: Serve as the primary staff liaison with Michigan Rehabilitation Services (MRS) and other participating agencies to assist students and families in understanding post-high school options.
    • Instructional Leadership: Provide professional development and coaching to staff regarding transition best practices including work-based learning and interagency collaboration. Attend Oakland Schools OCTA meetings monthly and report learning back to district administration.
    • Work-Based Learning: Coordinate the development of robust Work-Based Learning (WBL) programs at the high school and post-secondary level ensuring job site development and student supervision.
    • Data Management: Work with the ISD to collect state performance plan transition data (SPP #13 and #14) use of catamaran and maintain accurate confidential student case files.
    • Community Relations: Establish and maintain a robust network of school-community partnerships and serve on appropriate community advisory boards.
    • Student Support: Facilitate transition planning meetings with parents and students to identify post-school goals service needs and community resources.
    • Curriculum alignment: Assist with transition skills being incorporated into all levels K-post secondary

    EDUCATION and/or EXPERIENCE:Masters degree

    LANGUAGE MATHEMATICAL AND REASONING SKILLS:Ability to speak and write to apply mathematical concepts to practical to carry out instructions furnished in written or oral format and deal with problems involving several concrete variables in standardized situations.
