Position Overview

We are seeking a Licensed Master Social Worker (LMSW) for a 14-week contract assignment at a large acute care hospital in New York NY. The LMSW will provide comprehensive social work services to adult patients focusing on psychosocial assessments discharge planning crisis intervention and care coordination.

This role requires strong clinical judgment hospital-based experience and the ability to collaborate effectively within an interdisciplinary healthcare team.

Responsibilities

Conduct thorough psychosocial assessments for adult patients.
Develop coordinate and implement individualized discharge plans.
Provide crisis intervention and short-term supportive counseling.
Collaborate with physicians nurses case managers and other healthcare professionals to ensure coordinated patient care.
Facilitate referrals to community agencies and support services.
Advocate for patients to ensure access to appropriate care and resources.
Maintain accurate timely and compliant documentation in accordance with hospital policies and regulatory standards.

Required Qualifications

Masters Degree in Social Work (MSW) from an accredited program.
Active Licensed Master Social Worker (LMSW) license issued by NYSED (Primary Source Verification required).
Minimum one (1) year of LMSW experience.
Experience working with adults with mental illness.
Prior hospital-based social work experience required.
