Job Summary
Empath Health is seeking a compassionate Home Health Aideto join our interdisciplinary PACE team in Tampa FL. Under the direction and supervision of the Center Manager the Home Health Aide is responsible for providing and assisting participants with activities of daily living restorative and supportive care. The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain independent and safe in their own homes.
Relationship-Based Care with Real Impact: Build lasting connections with participants supporting their independence and mobilitythis is a meaningful alternative to inpatient rehab or traditional outpatient care.
Team-Driven Preventive Focus: Collaborate with physicians nurses social workers and other clinicians in a true interdisciplinary care model focused on prevention and holistic well-being.
What Youll Do
- Assist the day center supervisor and recreational therapist in providing therapeutic recreation programs tailored to the participants level of care.
- Assist with feeding and monitoring the participants during mealtime. Will report any dietary concerns to the day center supervisor and the dietician.
- Assist participants on and off the vans when they arrive and leave the center.
- Assist with encouraging participants to remain physically active within in attendance at the center.
- Report any concerns about a participants ambulation and mobility status to the Center Manager and Therapy team

What Youll Need
- High School diploma or equivalent required.
- Current Home Health Aide Certification required.
- Valid CPR certification from the American Red Cross or American Heart Association.
