Job Summary
Park Nicollet is looking to hire a Registered Nurse Transition Liaison to join our Homecare team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
The Transition Nurse Liaison will be responsible for coordinating the care of patients in Health Partners enterprise Hospital post-acute care network whose medical condition indicates that they will benefit from Home Care/Infusion Services. This position will assume primary coordination responsibility for Home Care/Infusion referrals to Methodist Home Care (MHC) and/or Park Nicollet Home Infusion (PNHI). The Transitional Nurse Liaison will work closely with pharmacy nursing social services physicians rehabilitation services patients and their families.
Work Schedule: M-F 8am-4:30pm
Required Qualifications:
- Education Experience or Equivalent Combination:
- Minimum of two years hospital-based acute care nursing experience.
- Two years experience in a home care setting.
- Licensure/ Registration/ Certification:
- Current Minnesota R.N.
- License BLS provider status in accordance with American Heart Association (AHA) guidelines
- Current MN drivers license
- Knowledge Skills and Abilities:
- Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
- Requires basic computer/keyboarding skills.
Preferred Qualifications:
- Education Experience or Equivalent Combination:
- BSN preferred.
- Previous hospital-based case management experience strongly desired
- Nursing experience in specialty areas including but not limited to oncology neurology or orthopedics.
- Knowledge Skills and Abilities:
- Knowledge of how hospital departments interact and how patients flow through a hospital setting.
- Knowledge of external post-acute and sub-acute health care providers/systems/options in the Twin Cities and greater Minnesota.
- Knowledge of word processing spreadsheet and presentation software desired.
