Position Summary The Licensed Practical Nurse (LPN) provides nursing care in a long-term care setting located within a hospital environment. Key Responsibilities -
Perform patient assessments and deliver assigned nursing treatments

Administer medications and monitor patient responses

Communicate pertinent patient information to physicians and nurse practitioners

Assist with admissions transfers and discharges
-
Support activities of daily living (ADLs)
-
Provide specialized care including:
-
Gastrostomy tube care
-
Urinary catheter care
-
Nebulized treatments
-
Wound vac management
-
CPAP care
-
Tracheostomy care
-
Document patient care accurately in the medical record
-
Carry out additional duties as assigned
Schedule Requirements -
Active LPN license (or ability to obtain prior to start)
-
Minimum 1 year of long-term care or related nursing experience
-
Current BLS/BCLS certification
-
Ability to work flexible schedules and float as needed
Additional Information -
Proof of flu vaccination required prior to assignment (may be completed during employee health clearance)

Candidate must complete pre-employment health requirements before orientation

Orientation and unit-specific training provided
