Job Summary
Empath Home Health a part of Empath Health is seeking highly motivated and compassionate Home Health PT Physical Therapists fulltime for our Fort Myers Team. Location: Home Care Visits in Lee County Sanibel Fort Myers
Schedule: Full time: M-F 40 Hours. Requires weekend/holiday/on call rotations
What youll Do
- Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
- Perform initial and ongoing assessments including OASIS assessments at appropriate time points to determine level of functioning and patients ability to perform activities of daily living.
- Develop and revise plans of care in consultation with the physician and other care team members which will enable patient to achieve realistic goals of mobility.
- Reports to the physician and designated agency representative patients reaction to treatment and any changes in the patients condition.

What Youll Need
- Licensure as a physical therapist in the state of Florida
- Valid FL drivers license
- Minimum of one (1) year experience in home health care preferred
