Hospice Admissions Nurse (RN) Location: Rockville MD
Job Type: Full-Time
Schedule: 4 days per week 10-hour shifts (8:30 AM 7:00 PM)
Pay Rate: $45/hour and up (based on experience)
Position Overview NurseAmerica is seeking a compassionate and experienced Hospice Admissions Nurse (RN) to join our dedicated hospice team serving patients throughout Montgomery County. This role focuses on conducting admissions initiating plans of care and delivering high-quality hospice and palliative services that promote dignity comfort and peace during end-of-life care.
The ideal candidate is clinically strong empathetic highly organized and comfortable leading meaningful conversations with patients and families during critical moments.
Key Responsibilities -
Conduct comprehensive initial assessments to determine patient eligibility and care needs

Obtain informed consent and coordinate admission into hospice services

Develop and review individualized plans of care in collaboration with physicians

Serve as a clinical resource for referral sources patients and family members
-
Facilitate end-of-life discussions and support decision-making with families and caregivers
-
Provide hospice and palliative care services in patients homes or residences
-
Maintain accurate documentation in EMR systems
-
Represent NurseAmerica professionally in all patient family and physician interactions
Qualifications -
Active Registered Nurse (RN) license State of Maryland

Minimum 3 years of acute care and/or hospice experience

CHPN certification preferred

Strong knowledge of end-of-life and hospice care principles

Excellent communication organization and customer service skills

Proficiency in Microsoft Office 365 and EMR systems

Experience with Netsmart/MyUnity or Homecare Homebase is a plus
-
Valid drivers license and reliable transportation for travel within Montgomery County
