What youll do

This is a community-based Registered Nurse role focused on delivering high-quality person-centred nursing care to older Australians living at home. You will work across both care management and office-based clinical functions supporting customers with complex and escalated needs.

Youll provide sound clinical assessment care planning and intensive case management within your scope of practice particularly for customers requiring high-level Support at Home reablement restorative and end-of-life care.

Working collaboratively within a multidisciplinary team you will guide care planning support service delivery teams with clinical expertise and ensure customers receive timely appropriate and evidence-based care.

Youll be trusted to make a valuable impact by:

  • Conducting comprehensive clinical assessments and reviews to inform high-quality care planning
  • Managing escalated and complex cases including restorative short-term and palliative care
  • Providing wound management expertise and clinical advice to multidisciplinary teams
  • Prioritising and managing a dynamic caseload in a fast-paced community environment
  • Supporting high clinical governance standards through accurate documentation and best-practice nursing care

To thrive in this role you will have:

  • Bachelor of Nursing and current AHPRA registration as a Registered Nurse
  • Minimum 3 years post-registration experience (acute hospital and/or community setting)
  • Strong experience in wound management and exposure to palliative or end-of-life care
  • Demonstrated ability to manage competing priorities and work autonomously
  • Current drivers licence and access to a fully insured vehicle

We would also love you to have:

  • Experience in Community Aged Care
  • Short Term Restorative Care / TCP experience
  • Experience in community-based restorative or reablement models
  • Confidence working within a multidisciplinary team
  • Resilience strong time management skills and a proactive solution-focused mindset
