Job Summary
Empath Health is seeking a compassionate CNA/Program Aide to join our interdisciplinary PACE team in Tampa FL. Under the direction and supervision of the Center Manager the CNA/Program Aide is responsible for providing and assisting participants with activities of daily living restorative and supportive care. The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain independent and safe in their own homes.
Relationship-Based Care with Real Impact: Build lasting connections with participants supporting their independence and mobilitythis is a meaningful alternative to inpatient rehab or traditional outpatient care.
Team-Driven Preventive Focus: Collaborate with physicians nurses social workers and other clinicians in a true interdisciplinary care model focused on prevention and holistic well-being.
What Youll Do
- Assist the day center supervisor and recreational therapist in providing therapeutic recreation programs tailored to the participants level of care.
- Assist with feeding and monitoring the participants during mealtime. Will report any dietary concerns to the day center supervisor and the dietician.
- Assist participants on and off the vans when they arrive and leave the center.
- Assist with encouraging participants to remain physically active within in attendance at the center.
- Report any concerns about a participants ambulation and mobility status to the Center Manager and Therapy team

What Youll Need
- Education and/or Experience: High School diploma or equivalent required. Current Certification as Certified Nursing Assistant (C.N.A) in the State of Florida
- Continuing Education: As required for licensure.
- Certifications:
- Current Certification as Certified Nursing Assistant (C.N.A) in the State of Florida required
- Home Health Aide certification with 75 hours completed preferred
- Valid CPR certification from the American Red Cross or American Heart Association
