Job Summary
Licensed Practical Nurse (LPN) Per Diem (Home Health)
Pay: $60 per visit
Location: Oahu Hawaii (must have reliable transportation we serve the entire island)
Schedule: Flexible / Per diem build your own caseload

skilled nursing visits in patients homes under RN direction
Assist with medication management and patient education
Perform wound care and routine skilled nursing tasks within scope
Communicate changes in patient condition to RN / clinical team
Document accurately and on time
Requirements
Current Hawaii LPN license
1 year clinical experience (home health preferred)
Strong communication and documentation skills
Valid drivers license reliable transportation
Ability to work independently in the field
