Overview
The Cook Family Health Center is the primary care medical practice operated for Cook Group company employees and their families.
The Pharmacist at the Cook Familty Health Center will dispense drugs prescribed by physicians and other health practitioners and provide education to patients about medications and their use.
Responsibilities
Provides clinical drug information to meet the needs of the patients nurses and physicians/providers
Reviews and interprets physicians/providers orders for appropriateness manages formulary to be mindful of effective drug therapies and cost efficiency
Dispenses prescriptions in accordance with clinic policy and in conformance with regulations and laws dealing with the class of prescription
Counsels patients about their medications and maintains an accurate up-to-date patient profile
Maintains necessary controls with respect to audited controlled drugs
Maintains up-to-date knowledge of the profession and assures the quality of professional service is maintained
Qualifications
Current Indiana Licensed Pharmacist
Doctorate of Pharmacy (PharmD) or Bachelors of Science in Pharmacy (prior to 2006) required
3-5 years recent experience as a Pharmacist
Microsoft Office suite (Word Excel Outlook) and pharmacy information systems proficiency
Physical Requirements:
Ability to walk & stand for long periods of time
Able to bend stoop kneel or crouch
Utilizes close visual acuity for working with computers etc.
Able to lift 20 pounds occasionally
