- Build deep rewarding relationships as the primary point of contact for your patients
- Work in a supportive collaborative Interdisciplinary Team environment

- Develop and manage individualized Plans of Care with patients families physicians and the IDG
- Coordinate all hospice services and ensure seamless care across settings
- Provide expert pain and symptom management
- Educate patients and families on the Medicare Hospice Benefit and hands-on care
- Supervise Home Health Aides and deliver high-quality care
- Complete timely documentation and participate in admissions
- Current Oklahoma Registered Nurse license
- At least 1 year of recent medical/surgical oncology home health or hospice experience
- Strong knowledge of pain and symptom management in terminally ill patients
- Excellent communication organization and problem-solving skills
- Valid drivers license reliable transportation and current auto insurance
- $2500 Sign-On Bonus paid out over the first 6 months
