Position Overview The Physical Therapist (PT) is a key member of the interdisciplinary team (IDT) responsible for care planning coordination and delivery of restorative and rehabilitative physical therapy services.
This role focuses on maximizing participant independence mobility and safety in a patient-centered collaborative care environment.

Key Responsibilities -
Conduct Physical Therapy assessments in the center and occasionally in participants homes

Define participant care needs goals and preferences

Actively participate in interdisciplinary team (IDT) meetings

Develop and implement individualized care plans

Deliver and document therapeutic interventions including:

Supervise Therapy Assistants and provide instruction for Functional Maintenance Programs (FMP)

Train staff participants and caregivers on proper use of therapeutic equipment
Minimum Qualifications -
Bachelors Degree in Physical Therapy (Masters or Doctorate preferred)

Valid California Physical Therapy License or proof of eligibility

Current CPR certification (preferred)

1 2 years of clinical experience

Experience with frail or elderly populations preferred
