Minimum Qualifications:

  • Current State of Michigan licensure as a TLLP LLMSW or LLPC.
  • Must meet Michigan PIHP/CMHSP Medicaid qualifications for a Mental Health Professional (MHP).
  • Valid Michigan drivers license required.
  • Knowledge of co-occurring disorders Stages of Change (Transtheoretical Model) phases of recovery and Motivational Interviewing/Motivational Enhancement Therapy.
  1. Assesses and evaluates the needs of ACT individuals and continues caseload contact to develop goals. Assists individuals in developing appropriate program plans to meet identified needs. Completes referral forms clinical and legal documentation necessary to obtain community based services for individuals. Advocates for new services as needed. Seeks out and develops community resources to meet the needs of assigned clients for activities and support services.
  2. Engages consumers and families in a welcoming hopeful empathic manner regardless of stage of readiness and phaseof treatment.
  3. Provides case management services for individuals families and/or guardians for support rehabilitation and/or crisis intervention purposes. Monitors and evaluates living situations school and/or program placements of individuals to determine consistency of treatment and progress. Assists in commitment procedures when necessary as well as voluntary admissions.
  4. Attends daily team meetings to review problems and plan solutions and completes meeting minutes on a rotation meetings with ACT psychiatrist at least twice per week to ensure continuity of individual care.
  5. For individuals hospitalized for psychiatric reasons the clinician will meet with the individual within 72 hours of the psychiatric admit.
  6. Assists individuals with medication compliance and distributes medication vials to individuals as ordered by the psychiatrist as well as eyes on meds activities to ensure compliance with treatment plan.
  7. Works within the rotation of ACT individuals to ensure that individual needs are met by all professional disciplines including transportation.
  8. Ensures that paperwork is maintained in accordance with Medicaid and Department of Community Health guidelines as well as Easterseals policies and procedures.
  9. Maintains up-to-date knowledge and understanding of Medicaid and ACT criteria and individual eligibility.
  10. Attends ACT 101 and 16 hours of IDDT training within 6 months of hire and yearly ACT training and IDDT training.
  11. Travels extensively to various locations within southern Oakland County to assess and evaluate the needs of individuals and develop program goals.
  12. Responsible for authorizing medically necessary services for all individuals.
  13. Adheres to the IDDT fidelity model of substance abuse treatment.
  14. Analyzes clinical histories for diagnostic purposes and develops individualized treatment plans for individuals.
  15. Conducts clinical assessments and provides individual group conjoint or family psychotherapy to carry out the treatment plans to attain individual goals.
  16. Performs assessments for diagnostic purposes interviews individuals and others related to the case and reviews applicable case history to determine needs.
  17. Develops treatment plans for SA MH and COD that are individualized and Person Centered and reflect proper treatment matching. Specific goals objectives and Stage/Phase specific treatment interventions are identified for each assessment needed. If differed the reason for doing so is identified. Community resources and supports are prioritized as a strategy to assist individuals with meeting their goals.
  18. Assist individuals with identifying changes in behavior attitude feelings and thoughts that are triggers for in the development of a relapse prevention/crisis plan utilizing personal community and provider supports.
  19. Demonstrate the ability to identify stage change and use interventions consistent with stage of treatment therapeutic counseling on a limited short term basis and offers crisis intervention as needed. Evaluates the progress of individuals on an on going basis.
  20. Screen for co-occurring mental health and substance abuse disorders. Implement integrated strength based assessment to assess the presence onset symptoms and patterns of substance use behavioral and psychiatric disorders. Assess for acute needs and risk and provide/arrange for appropriate stage wise the presence of two disorders evaluation will be made of the interaction between the two as well as the relationship of each to significant life events. Demonstrate the knowledge of diagnostic criteria for psychiatric and substance use disorders.
  21. Demonstrates knowledge of principles of trauma informed care.
  22. Willingness to support and assist individuals with trauma related issues.
