Job Summary

Job Title: Home Health Medical Social Worker (MSW)
Location: Kansas City MO & Surrounding Areas
Schedule: Monday Friday 8:00am 5:00pm (start/end times may vary); no mandatory on-call
Compensation: $58000 $67000 annually

A growing healthcare organization is seeking a compassionate and motivated Home Health Medical Social Worker to join their care team serving Kansas City MO and surrounding counties. This role focuses on collaborating with interdisciplinary teams to deliver comprehensive patient-centered care in a home health setting.

Key Responsibilities:

Collaborate with the care team to develop and manage patient care plans
Provide social work support to patients and families in home health settings
Travel to patient homes within assigned territory (approximately 20% of the day)
Complete accurate and timely documentation in EMR systems
Support patients across Platte Clay Leavenworth Wyandotte Johnson Miami Jackson and Cass counties

Required Qualifications:

Masters degree in Social Work from an accredited program
Active and valid MSW license in Missouri or Kansas (in good standing)
Minimum one (1) year post-graduate social work experience in a healthcare setting
Experience using Electronic Medical Records (EMR) including TruChart EHR or similar systems
Valid drivers license reliable transportation and active auto insurance
Ability to meet documentation deadlines consistently
Strong time management and ability to self-schedule effectively
Comfortable using tablets mobile devices and EMR software

Additional Information:

No mandatory on-call requirements
Two paid holidays per year (one summer one winter) plus three floating holidays
