Upon Offer your agency will be REQUIRED to sign the staffing agreement through ShiftWise to book the candidate. **Rate is open to bids please note rate on coversheet** Physical Therapy Assistant - Hartford Home Care Division Address of facility: 34 Ledgebrook Drive Mansfield CT Schedule: M-F 8am-4:30pm. No call. 1 yr Home care experience required Certifications required: PTA license Connecticut license required before starting BLS.

Shift: 40 Hours. M-F 8am-4:30pm. No call.
