You will serve adults with:
Severe mental illness
Co-occurring substance use disorders
You will work inside an Assertive Community Treatment Team (ACTT) model services delivered in homes and community settings not an office-only role.
Your Core ResponsibilitiesComplete substance use and co-occurring disorder assessments
Build individualized treatment plans with the ACTT team
Deliver 1:1 substance use counseling using:
Motivational Interviewing
Cognitive Behavioral Therapy
Relapse prevention strategies
Provide crisis response (rotating 24/7 coverage)
Deliver services in homes shelters hospitals and community settings
Document services to meet Medicaid standards
Facilitate person-centered planning meetings
Support anger management and behavioral stabilization
Coordinate care with psychiatry nursing employment specialists and peer support
You will spend the majority of your time in the community rather than behind a desk.
You already meet North Carolina Qualified Professional standards under 10A NCAC 27.G.0104.
You hold one of the following active credentials:
CCS
CCAS
CSAC
You have direct experience with:
Dual diagnosis treatment
Crisis intervention
24/7 on-call rotation
You are comfortable entering homes de-escalating tense situations and collaborating with a 610 person multidisciplinary team.
Drive daily to client locations
Lift up to 25 lbs
Work in homes shelters hospitals and office settings
Participate in evening/weekend/holiday on-call rotation
You may encounter high-risk or volatile situations. Crisis response training is required.
