Job Summary

Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team.
Schedule 8pm - 8am Week 1 & 2: Tue Wed Thu. Coverage area to include Manatee Charlotte De Soto and Sarasota County.
What you do will do:
- Under the direction of the RN case manager provides continuous bed-side clinical observations prescribed care physical and emotional support to patients as directed in plan of care reporting significant changes in patient condition and family needs as needed.
- In collaboration with the RN accepts and processes physician orders to maximize patient comfort.
- Crisis Care nurses are cross-trained to Hospice Houses and Triage to assist with patient care needs when cases are pending.
Requirements:
- Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
- Minimum one year experience in an acute care setting providing direct patient care; previous experience as a hospice LPN preferred.
