POSITION SUMMARY:
The Inpatient Clinical Pharmacist is an integral member of the interdisciplinary healthcare team responsible for optimizing medication therapy and ensuring safe effective and patientcentered pharmaceutical care. This role combines direct clinical service operational pharmacy support education and continuous quality improvement.
KEY RESPONSIBILITIES
Provide Direct Patient Care:
Develop recommend and monitor individualized pharmacotherapeutic regimens using expertise in pharmacology pathophysiology and pharmacokinetics. Participate in patient care rounds and collaborate closely with physicians nurses and other healthcare professionals to ensure optimal medication outcomes.
Deliver Clinical Pharmacy Services:
Offer timely drug information therapeutic recommendations and evidencebased guidance to the medical staff. Support antimicrobial stewardship anticoagulation management and other clinical initiatives as assigned.
Staffing Responsibilities:
Rotate through decentralized clinical pharmacy services and centralized pharmacy operations. This position primarily rotates between department day and evening shifts hours of 6:00 a.m. and 11:00 p.m. Occasional overnight coverage may be requested; such instances are infrequent. Perform order verification medication distribution and sterile and nonsterile compounding in accordance with hospital policies and regulatory standards.
Medication Preparation & Dispensing:
Accurately compound package label and dispense medications IV solutions and specialized preparations. Ensure compliance with USP <797>/<800> and maintain a high standard of safety and quality.
Supervision & Delegation:
Provide oversight guidance and support to pharmacy technicians and pharmacy interns in daily operations. Ensure appropriate task delegation and adherence to workflow standards.
Education & Mentorship:
Serve as a resource and preceptor for pharmacy students residents and learners from other health professions. Contribute to the development of educational programs and training activities.
Quality Improvement & Professional Development:
Participate in departmental performance improvement projects and hospital quality initiatives. Pursue ongoing professional growth to meet organizational departmental and personal advancement goals.
POSITION QUALIFICATIONS
Education & Licensure
- Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) or other advanced degree preferred.
- Must be licensed or eligible for licensure as a pharmacist in the State of Michigan.
- Strongly preferred: completion of an ASHPaccredited residency (PGY1 and/or PGY2)
Interpersonal & Communication Skills
- Demonstrated ability to interact effectively and professionally with patients families healthcare team members support staff medical staff and the public.
- Strong verbal and written communication skills with the ability to convey clinical information clearly and compassionately.
Safety & Compliance
- Proven ability to perform all duties in a safe accurate and compliant manner while adhering to hospital policies procedures and regulatory standards in the delivery of patient care.
Clinical Judgment & ProblemSolving
- Ability to identify medicationrelated issues and patient care concerns recognize situations outside the pharmacists scope of responsibility and initiate appropriate followup or escalation.
Adaptability & Workflow Management
- Demonstrated flexibility in adjusting work priorities in collaboration with supervisors based on patient care needs staffing demands or departmental changes.
- Ability to thrive in a dynamic fastpaced environment requiring significant adaptability.
