Pay: $2634.00/week
Completion Bonus: $2500 per contract (up to $7500 after three contracts)
Job Description
ATC Healthcare is seeking an experienced Home Health Physical Therapist (PT) for a travel assignment in Novato California. This role provides in-home physical therapy services to adult and geriatric patients focusing on functional mobility rehabilitation safety and independence in the home setting.
The PT will deliver comprehensive home-based care develop individualized treatment plans perform functional assessments and collaborate with interdisciplinary teams to support recovery and long-term patient outcomes. This position requires strong autonomy critical thinking and the ability to manage complex caseloads independently in a community-based care environment.
Key Responsibilities
- Perform in-home physical therapy evaluations and reassessments.
- Develop and implement individualized treatment plans.
- Deliver skilled therapy focused on mobility strength balance and function.
- Conduct safety and environmental assessments in patient homes.
- Educate patients and families on exercises safety and care plans.
- Complete OASIS documentation and care coordination.
- Manage discharge planning and long-term goals.
- Collaborate with interdisciplinary care teams (PT social services home health staff).
- Maintain accurate and timely clinical documentation.
Qualifications
- Graduate of an accredited physical therapy program.
- Active California Physical Therapist license (required at submission).
- BLS certification (AHA or American Red Cross).
- Valid Drivers License (Class C) and active automobile insurance required.
- Minimum 2 years of experience required (5 years recent relevant experience strongly preferred).
- Travel experience required.
- Home Health experience preferred.
- Strong independent clinical judgment and problem-solving skills.
- Excellent communication organization and time management abilities.
- Proficient with required technology and documentation systems.
