Home Health Registered Nurse (RN)

Weekend Schedule Lafayette IN Flexible Options Available

Weekends only weekdays free. Provide 1:1 patient care in the home no call lights or 6-patient assignments. New to home health We provide training and ride-along support.

We are hiring a Weekend Home Health RN in the Lafayette area. This role is ideal for nurses who prefer fewer weekday obligations want a consistent routine or are balancing family school or a second job.

No home health experience We will train you. Many of our best nurses came from hospital rehab and skilled nursing settings.

What Youll Do

As a Home Health RN youll care for patients in their homes and help them safely manage their health conditions.

Responsibilities include:

• Completing start-of-care and routine nursing visits

• Assessing patient condition and developing care plans

• Providing medication and disease-management education

• Monitoring progress and preventing hospital readmissions

• Coordinating care with physicians therapists and care team members

• Identifying changes in condition and communicating appropriately

• Documenting visits in the electronic medical record

• Maintaining HIPAA and patient safety standards

Training & Support

You will not be sent out unprepared. We provide:

• Structured onboarding

• Ride-along visits with experienced clinicians

• EMR training

• Ongoing clinical mentorship and leadership support

What Were Looking For

• Active unrestricted RN license

• 1 year of nursing experience (hospital SNF rehab clinic etc.)

• Willingness to work independently in the field

• Reliable vehicle valid drivers license and auto insurance

• Ability to travel within the Lafayette coverage area

• Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.

Preferred (not required):

• Home Health or Hospice experience

• HomeCare HomeBase (HCHB) experience

• ASN or BSN
