Hospice of South Texas (HOST) is seeking a compassionate Medical Social Worker to support patients and families through end-of-life care. This role is open to LBSW LMSW or LCSW candidates with at least one year of healthcare experience.

Essential Functions
- Complete psychosocial assessments for hospice patients and families
- Provide emotional support counseling and resource coordination
- Participate in interdisciplinary team meetings
- Assist with care planning coping strategies and end-of-life support
- Maintain timely and accurate clinical documentation
- Collaborate with nursing chaplaincy and bereavement teams
Qualifications
Required
- LBSW LMSW or LCSW (Texas)
- Minimum of one (1) year of experience in a healthcare setting (regulatory requirement)
- Strong communication and interpersonal skills
- Ability to manage emotionally complex situations with professionalism
Preferred
- Experience in hospice home health hospital SNF dialysis oncology or behavioral health settings
