Be the steady voice patients and families rely on during their most vulnerable moments! We are seeking a compassionate Social Worker to join our Oncology Team!
MWF 7a-4p
Every 4th weekend on call (telephone availability only)
Oncology experience preferred
Inpatient social work experience preferred
LMSW required
Masters Degree required
The Opportunity
Responsible and accountable for assessment of psychological social emotional cultural and financial needs and the resulting treatment that impacts patients health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness crisis intervention and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity health system and community.
